Referrals
GP referrals
All NHS GP referrals must be submitted using the NHS e-Referral Service (e-RS). Referrals sent by email or post from NHS GPs will not be processed.
We accept referrals from consultants and GPs. Please complete the below referral form and send it to us via email.

Referral criteria
The assessment process is to determine whether your patient is suitable for a pain management programme or self-management approach, neuromodulation device or treatment.
Inclusion:
- be over 18
- have been in pain for more than 6 months
- have pain that is severe enough to prevent them working or taking part in other daily activities
- not need or have surgery and other treatments planned that might impact their ability to take part
- be open and committed to the programme and willing to take part in a group
Exclusion:
- has no significant impact on their functioning
- cannot control their use of injectable analgesia, medicated lollies or oral liquid opioids
- has addiction or substance abuse problems (including alcohol and cannabis)
- cannot move around on their own or fully self-care, a case management service is available for patients who need tests done before attending
- has overriding complexities that will make engagement difficult. These may need addressing either before or instead.
- has moderate to severe cognitive impairment
- struggles to participate in English - individual sessions may be offered instead
- are under 18
