Making a referral

We accept referrals from GPs and hospital consultants in:

• Lambeth
• Southwark
• Lewisham
• Bexley
• Bromley
• parts of Croydon and Kent
• other areas where GP or CCG funding has been agreed

To make a referral, please complete the amputee referral form and send back to us by email or post. 

Emergency referrals

We offer an emergency walk-in service for amputees, Monday to Friday, 8.30am to 4.30pm. You do not need to book an appointment in advance.

This service is for patients with a broken prosthesis or an injury to the residual limb. Patients should call before arriving so that we can help with any access arrangements and minimise waiting times.

If your patient has been under the care of another prosthetics centre, please tell us which one and arrange for them to send us a copy of the patient's notes.
