In response to the coronavirus (COVID-19) outbreak, please read our advice and information before sending any referrals.


Referral contacts

How to make a referral

We accept referrals from the patient, relatives, GPs, care home staff, community nurses, hospital staff and social workers.

Please download and send either a Southwark referral form (Word 111Kb) or a Lambeth referral form (Word 196Kb) to the relevant contacts at the top of this page. Please provide as much information as possible on the form.

Inclusion criteria

We accept self-referrals and referrals from relatives, GPs, care home staff, community nurses, hospital staff and social workers for adults of any age.

Goals for rehabilitation must be identified, for example: to improve mobility, return to previous activities.

Intervention is time limited and focused on the achievement of goals and self-management of long-term conditions.

Referrals are not accepted for equipment only. Instead, these referrals should be sent to the occupational therapy departments of Lambeth social services or Southwark social services.

Exclusion criteria

  • Conditions with a primary neurological diagnosis.
  • Clients requiring an urgent response.
