Pulmonary rehabilitation referrals

For people with lung conditions experiencing breathlessness

Coronavirus: physiotherapy update

In response to the coronavirus (COVID-19) outbreak, please read our advice and information before sending any referrals.

We accept referral letters from:

  • GPs and practice nurses
  • doctors, nurses and physiotherapists working in outpatient chest clinics or on the inpatient wards at Guy’s and St Thomas’ Hospital
  • other specialist healthcare professionals working in the community such as in the @home team.

GP referral

We accept referrals from GPs and practice nurses.

To refer to pulmonary rehabilitation from primary care, please use the NHS e-Referral Service.

Patient self-referral

Please speak to your GP, practice nurse or chest doctor if you would like to be referred to pulmonary rehabilitation.

Referral criteria

Patients must be 16 years or over with breathlessness on exertion confirmed to be due to a respiratory cause. The patient should have had lung function tests within the last two years, which should be included with the referral.
