Tissue viability service referrals

Helping to prevent pressure ulcers and advising on wound management

Referral contacts


We accept referrals from GPs, district nurses and nurse practitioners for patients living, or registered to a GP in Lambeth and Southwark via the tissue viability referral email address. Please complete the referral form and if possible also attach a wound photograph.

  • Inclusion criteria

    The service supports anyone who has a wound, or is at risk of developing a wound. 

  • Exclusion criteria

    We do not generally accept self-referrals. However, if you are not currently receiving care from a health care professional and require further information please contact us.

  • Area of referrals

    A patient currently at one of our hospitals or someone who lives within the London boroughs of Lambeth and Southwark and is registered to a Lambeth or Southwark GP.
