How to get an appointment

You can self-refer to our service.

We also accept referrals from GPs and other health professionals.

Before your appointment

Please bring:

  • any medicines you are taking
  • letters relating to your care
  • a relative or friend 

During your appointment

Your first appointment can take up to 1 hour. You'll see a nurse who will:

  • ask you questions about your general health
  • discuss your care plan 

The nurse will also ask you how you would like to be contacted for any follow-up appointments, for example:

  • by text
  • by telephone
  • by letter

Sometimes we may have student nurses at your appointment. We'll ask if you are happy for them to be there. If you would like to have someone with you during your consultation, please tell a member of our team.

Maintaining your confidentiality is important to us. We'll not tell other people about your visit.

We'll only pass on confidential information without your permission to protect you or someone else from serious harm. We'll always discuss this with you first.

After your appointment

We'll arrange any follow-up appointments with you after your first visit.

If you agree, we'll send a letter to your GP and any other health professional involved in your care.
