Lambeth diabetes intermediate care team

The Lambeth diabetes intermediate care team provides diabetes care to people in the community.

Our aim is to empower you to self-manage your diabetes.

We also:

support Lambeth GP practices to provide high quality diabetes care
work with other services including community nursing and secondary care diabetes services

The services we provide include:

5 specialist clinics every week across Lambeth
early kidney disease in type 2 diabetes clinic
telephone and email advice and support to primary and community care teams
telephone and email support to people with diabetes attending our service
home visits to people who are housebound
virtual diabetes clinics in GP practices
a diabetes education and self management for ongoing and newly diagnosed (Desmond) programme
food and diabetes sessions for people with type 2 diabetes
insulin and GLP1 start groups
STEPS (type 2 diabetes prevention programme)
