Learning disability community health

We accept:

• self-referrals
• referrals from the patient's carers
• referrals from GPs or other health professionals
• referrals from schools, colleges and social workers

To refer, please complete the referral form and email it to us.

We'll respond to routine referrals within 2 working days.

We'll respond to high-risk referrals (eating, drinking, breathing and falls) on the same day.
