Making a referral

All nuclear medicine and DEXA imaging tests need an appointment. We do not accept walk-ins.

Requesting a nuclear medicine scan

To make a referral for a nuclear medicine scan, please:

1. download the nuclear medicine request form
2. make sure all fields are completed
3. email the completed form to us

Requesting a DEXA scan

To make a referral for a DEXA scan, please:

1. download the DEXA request form
2. make sure all fields are completed
3. email the completed form to us

Incomplete or illegible forms will be returned and may delay the booking.

You can attach documents to the e-RS advice request. This may include diagnostic results and scanned images, such as ECGs. Providers can also respond with attachments.
