Your rights
Under data protection law, you have the right to:
- know how we use your information
- request a copy of the information we hold about you
- ask us to correct information that is inaccurate or incomplete
- object to certain uses of your information in some circumstances
- request that some uses of your information are restricted
- receive a copy of information you have provided to us in a reusable electronic format in certain circumstances
Some of these rights may be limited where information is needed for your care, to protect the rights of others, or where we are required by law to retain or use it.
We do not normally make decisions about you using fully automated processes without a member of staff being involved. If this ever applies, you will be informed and have the right to request a review of the decision.
Request a copy of your information
If you would like to see your health records you can ask the clinician who is treating you, or another member of staff.
If you do not understand parts of it, they will be able to explain it.
Apply for your health records
Please email, write a letter or complete an application form if you would like a copy of:
- your health records
- the health records for another living person
- the health records for someone who has died

Where we process data about you, you can request to receive a copy of the data free of charge. In some circumstances a fee may be charged, for example if repeated requests are made.
Exercising your rights
To exercise any of your other data protection rights, please contact our Information Governance team or Data Protection Officer.
Requests can be made verbally or in writing. We may ask for information to help identify you and understand the information you are requesting.
We aim to respond to requests within 1 calendar month. If we need more time, we will let you know and explain why.
Keeping your information safe
We take the security of your information seriously and have measures in place to protect it from unauthorised access, loss or misuse.
These include:
- secure IT systems
- access controls
- staff training
- monitoring of how information is accessed and used
We regularly review our security arrangements and only allow authorised staff to access information they need to do their jobs.
If a data breach occurs, we will investigate it and report it to the Information Commissioner's Office (ICO) where required by law.
How long we keep your information
We only keep personal information for as long as necessary and in line with NHS records management requirements and legal obligations.
