We'll invite you to sign up to MyChart if you have care with us.
To create a MyChart account you will need an activation code. We might:
- send you a code in an appointment letter, text or email from us
- give you a code before or during your appointment
- give you a code at a check-in kiosk when you come for an appointment
This process can be different for parents or carers, and young people under 18. Please speak to your healthcare team at the next appointment to ask for an account.
Opening MyChart for the first time
You can use the MyChart app on your mobile, tablet or computer.
From your internet browser
Visit mychart.gstt.nhs.uk or open MyChart.
Download the app
Download the MyChart app on your mobile or tablet using App Store or Google Play.
Open the app when the download is finished and select Guy's and St Thomas' NHS Foundation Trust from the list.
Completing your sign up
When you have opened MyChart, click 'new user' and enter your activation code. You will also need to add some personal details to confirm it’s you.
MyChart will ask if you want to set up two-step authentication. This is a security setting to keep your health record safe. It will check your contact details are right and ask you to set up a username and password. Choose something secure and memorable.
You'll be asked if you want notifications sent to your phone and your email. Finally, you will need to read and sign the terms and conditions.
After this you should be logged in to MyChart.
If your phone supports face or fingerprint ID you'll be offered the option to switch this on for your MyChart account. We recommend that you do this.
What to expect when you log in
MyChart only shows information about your care at hospital and community services at Guy's and St Thomas' and King's College Hospital NHS Foundation Trust. This is because we launched MyChart and our patient electronic health record system together.
It does not show records held by your GP or other providers, such as the information you might see on the NHS app.
MyChart also does not show any historical records from before October 2023, when the app launched. Once you sign up for an account, you'll start seeing appointments, notes and information in the app as you have your care with us.
Sharing access
You can give another person ongoing access to your MyChart account using 'proxy access'.
A trusted adult with proxy access can help you manage your care at our hospitals or in the community. They'll see your health information on MyChart, including your appointments, health conditions and medicines.
Giving someone proxy access lets them see most of your hospital health records, and should only be used if you are completely comfortable with sharing this information.
People with proxy access will not be able to view the part of your record containing your medical, family and social history on MyChart. Proxy access will not routinely let them view information on your MyChart account about:
- sexual and reproductive health
- HIV
- termination of pregnancy (abortion)
- gender identity development services
There might also be information marked as 'sensitive' by your clinician, such as psychology notes, which will not be visible in MyChart at all. This means neither you or anyone with proxy access will see any information marked as sensitive.
Please be aware, some information that can be viewed by someone with proxy access, such as the name of a clinician, could risk identifying information about your health or care. This includes historical records. If you have any concerns about this, please speak to your healthcare team and take caution before granting proxy access.
Personal information that you can update on MyChart, but is hidden from people with proxy access, is your:
- gender identity
- sex assigned at birth
- sexual preference
- preferred name
How to give someone proxy access
If you are 18 or over, and the person you want to give access to already has their own MyChart account at Guy's and St Thomas' or King's College Hospital, you can add them using the app.
If they do not have an account, but are or have been a patient at these NHS trusts, they can sign up for their own MyChart account but must have an account before you can invite them.
To give someone else with a MyChart account at our hospitals proxy access:
- log in to MyChart
- click 'Sharing Hub' from the menu on the left
- click 'Family, friends or a carer'
- click 'Give someone ongoing access' and click the plus button
- enter the details of the person you want to give access to
- send the invite
They should receive an invite by email to sign up as your MyChart proxy. When they've confirmed some details, they will be able to log in to their MyChart account and see your health record.
If the person you want to add as a proxy is not a patient with a MyChart account, you can ask a healthcare professional to set up proxy access in clinic.
If you care for an adult without capacity
If you are a carer for someone who lacks capacity to make decisions, or if you have lasting power of attorney, you might want to use MyChart to keep up to date with the care of the patient.
Speak to their healthcare team to ask for proxy access to a patient’s health record. They can give you an activation code.
Removing proxy access
You can change who has proxy access to your information by logging in to MyChart.
- Log in to MyChart
- Click 'Sharing Hub' from the menu on the left
- Select 'Manage proxy access' at the bottom of the page
- Click 'Remove' for the person you want to stop having access

MyChart is not for urgent problems or emergencies. If you have a serious medical problem and need help, call 999 or go to your nearest emergency department (A&E).
