Intermediate care Lambeth

We provide short-term support and advice if you find it difficult to carry out day-to-day tasks due to a:

  • recent illness
  • hospital admission
  • change of circumstances
  • gradual loss of ability over time

We support you from your own home to live as independently as possible by:

  • increasing your confidence and ability to move around your home and in the community
  • increasing your general fitness
  • improving your strength and balance and reducing your risk of falls
  • helping you to manage your daily living, such as personal care, and leisure activities
  • helping you to take part in roles such as a parent or carer, or return to work

We work together with Lambeth adult social care. Our team includes:

  • occupational therapists - helping you to perform daily activities, develop skills and take part in activities
  • physiotherapists - helping you manage your health condition and physical wellbeing
  • nurses - helping you understand your health condition, manage your condition and get the care you need to improve your health
  • rehabilitation support workers - supporting you with your daily exercise programme and/or daily living activities
  • social workers - assessing your day-to-day needs and providing support and advice

Research and clinical trials

Research is vital to improving the care that you receive when you're unwell. You can help improve healthcare by taking part in research studies at our Trust. During your appointment, ask your healthcare professional about research. They'll be happy to tell you about research studies you could be eligible to join.
