Your appointment
We provide face-to-face support in your home.
How to get an appointment
Your GP can refer you if they have concerns about your mobility and rehabilitation.
If you are already in hospital, the therapy teams who have seen you during your stay will refer you.

We will contact the relevant emergency services if we cannot contact you and we're concerned about your health or safety. In extreme circumstances, this may involve gaining entry to your home to make sure you are safe and well.
During your appointment
We will look at what activities you used to do and what you would like to do again. We will agree a plan with you and design a timetable of activities to achieve these goals. Treatment can last between 1 and 6 weeks.
It is important for us to know what you:
- used to do before you became unwell and/or needing to stay in hospital
- can do now
- want to be able to do
- are likely to achieve during your rehabilitation

We will visit often to check how you are progressing. As you become more independent, we will adjust the support we give you. We may provide you with equipment to make performing daily tasks easier and safer. We can also help you to access other community services.
We are a 7-day service and may visit between 7:30am and 10pm. We will try to come at a time that is best for you, but, this may not always be possible.
Please be aware we use public transport and do many visits per day. We will be in touch if we will be late or if we need to cancel your appointment.
We will end the service if you are unable to take part due to:
- your own choice
- medical issues that stop you from taking part
- having to stay in hospital for more than 72 hours
- not working with the team to meet your goals
- drug or alcohol misuse
- repeatedly not being available when we have arranged to visit you
After your appointment
If your care lasts more than 6 weeks, you may have to pay for treatment.
Towards the end of your treatment, the team will assess whether you need ongoing care from Lambeth Adult Social Care.
Treatment is non-chargeable for up to 6 weeks.
During this non-chargeable treatment, a financial assessment will be carried out if you need further support. This is to see if you qualify for financial help from Lambeth Council towards the cost of further treatment.
This is calculated by reviewing your level of savings and income.
Your social worker or assessor will explain your financial assessment and the charging policy. They will make sure you have all the information you need to help you make a decision about your ongoing care.
