Community foot health (podiatry)

Making a referral

Patients must be registered with a GP based in Lambeth or Southwark.

We accept referrals from health professionals or self-referrals. We also accept referrals from friends, carers or relatives on the patient's behalf.

Referrals will be triaged according to priority and placed on a waiting list. We'll contact the patient, by phone or in writing, inviting them to make their assessment appointment.

GP referrals

Please send your referral using the NHS e-Referral Service (e-RS).

Non GP referrals, self-referrals and referrals from friends, carers and relatives

Please complete the community foot health (podiatry) referral form and send by email or post.

Home visits

For a home visit, you must be referred by a health professional.

Urgent referrals

Our emergency foot clinics are available to patients registered with GP surgeries in Lambeth or Southwark if they meet the emergency clinic acceptance criteria.
