Referrals
Making a referral
We accept referrals from GPs and other hospitals (tertiary referrals).
GP referrals
All NHS GP referrals must be submitted using the NHS e-Referral Service (e-RS). Referrals sent by email or post from NHS GPs will not be processed.
If you need advice about a patient, please use advice and guidance on e-RS.
Tertiary and non GP referrals
Please email or post your referral.
Please provide as much information as possible so we can book your patient into the best clinic for them. We may contact your patient to check their needs before offering an appointment.
Referrals from other hospitals must include a completed inter-provider minimum data set. Make sure to send this within 48 hours of referring a patient.
2-week wait (2WW) referrals
You must submit your referral using e-RS. We won't accept referrals sent by email or post.
Non GPs should email their 2WW referral to [email protected].

Emergency referrals
If you want to make an emergency referral or need advice, please call our switchboard on 020 7188 7188 and ask for the duty diabetes and endocrinology registrar.
Southwark community diabetes referrals
Referrals to our Southwark community diabetes team must be made using e-RS.
Please complete the Southwark community diabetes referral form and submit it on e-RS.
We work with GPs and practice nurses to provide advice on diabetes management. Please contact us for advice.
