Making a referral

We accept referrals from GPs and other health professionals.

Please send your referral by email.

Referral criteria

We mostly see patients over the age of 65, but we also see younger patients who are medically complex.

We give priority to patients being referred before cancer or urgent surgery.

Urgent referrals

If a patient needs to be seen urgently, please contact us.
