Making a referral

We accept referrals from:

• GPs
• specialist health professionals

How to refer

Please complete the vascular anomalies referral form and email it to us. Please specify on the form if this is for a clinic review or a multidisciplinary team (MDT) discussion only.

When referring a patient with imaging, please make sure images are available to us on the image exchange portal (IEP).

You will need to coordinate with your picture archiving and communication system (PACS) team or the people responsible for IEP tasks.

When you email your referral form, please also confirm the images are on the IEP.

Our coordinators will make sure the images are available to our team.

Urgent referrals

Referrals should be referred using the details above. Please mark you referral as urgent and our clinical team will review.
