Making a referral

We accept referrals from:

• GPs
• district nurses
• nurse practitioners

Your patient must be living, or registered with a GP, in Lambeth and Southwark. They must:

• have a wound, or
• be at risk of developing a wound

Please complete the relevant referral form and email it to us. If possible, attach a picture of the wound.
