Referrals

Making a referral

We accept referrals from GPs.

GP referrals

All NHS GP referrals must be submitted using the NHS e-Referral Service (e-RS). Referrals sent by email or post from NHS GPs will not be processed.

In e-RS, please search "diabetic medicine" and then "general diabetic management".

A copy of the Lambeth diabetes intermediate care team referral form can be found on DXS. This  should be attached on e-RS.

You can use advice and guidance on e-RS if you need advice about making a referral.

Urgent referrals

Please call us, Monday to Friday, 9am to 5pm for advice.

If appropriate, we will see urgent referrals within 24 to 48 hours.

Acceptance criteria

We accept referrals for people:

  • with type 2 diabetes optimised on 2 or more oral agents (or insulin) with HbA1c >58mmol/mol, and need one or more of the following:
    • intensification of treatment with additional oral medication
    • initiation of injectable therapy (insulin/GLP-1)
    • with severe and/or frequent hypoglycaemia

Please note:

  • in patients with frailty, consider referring if HbA1c is >75mmol/mol and/or symptomatic of hyperglycaemia
  • if your patient is housebound and needs assistance in medications administration and/or blood glucose monitoring from district nurses, we will only accept the referral if your patient is already under the care of the district nursing team

Diabetic kidney disease and or cardiovascular prevention:

  • advice and guidance on SGLT-2 inhibitor for cardio-renal protection
  • diabetic kidney disease (eGFR 30-60 ml/min or ACR >30mg/mmol despite treatment)
  • diabetes advice for people with type 2 diabetes with known heart failure or cardiovascular disease
  • type 2 diabetes age 20 to 40 years with hypertension or evidence of diabetic eye disease

Exclusion criteria

We do not accept referrals if:

  • the patient has established type 1 diabetes
  • the patient is acutely ill
  • the patient is newly diagnosed, with BMI<22 and symptomatic (polyuria, polydipsia, weight loss or increased urine/blood ketones)
  • the patient is pregnant
  • the patient is under 18 years of age
  • eGFR < 30
  • non diabetic kidney disease
  • papid fall in eGFR (>15ml/min/year)
