Referrals

Making a referral

We accept referrals from:

  • GPs
  • other health professionals

GP referrals

All NHS GP referrals must be submitted using the NHS e-Referral Service (e-RS). Referrals sent by email or post from NHS GPs will not be processed.

Non-GP referrals

Please email or post your referral letter to us.

Urgent referrals

Please mark your referral as urgent. We will review your referral and prioritise urgent referrals.

Emergency referrals

Patients should attend their local emergency department (A&E) if they have symptoms of:

  • a new blood clot, or
  • bleeding

Pulmonary embolism lysis team referrals

If your patient is high-intermediate risk or has a high-risk pulmonary embolism and may need catheter-directed thrombolysis or thrombectomy, please contact the on-call haemostasis and thrombosis doctor during working hours.

Please phone 020 7188 7188 and ask to speak to the on-call haemostasis and thrombosis registrar or consultant. The pulmonary embolism lysis team (PELT) will review your referral.

Please make sure all clinical details, blood tests and image results are available. Imaging scans need to be sent to us using IEP. The on-call doctor will send you a referral form to complete. Alternatively, please download and complete the pulmonary embolism lysis team (PELT) referral form.
